Aline Vital Care LLC is required by law to maintain the privacy and security of your protected health information (PHI). This Notice of Privacy Practices applies to all records of your care generated by our agency, whether made by agency personnel or your personal doctor.

If you believe your privacy rights have been violated, you can file a complaint with us or with the Secretary of the Department of Health and Human Services. We will not retaliate against you for filing a complaint.
